Welcome to the transfer team. We are an integrated team of Health and Social care staff. We provide one point of access for services to all patients on adult wards, MAU and accident and emergency at Tameside General Hospital.
We offer health and social care assessment and provision. By providing timely nursing and / or social assessments to enable patients safe discharge from hospital. This could be to intermediate care or back home with primary care (community services?) and or social care support.
We Provide access to the following:
Our registered nurses, healthcare assistant practitioners and qualified social workers and assessors are experts in discharge planning, with the aim to provide a timely response, to facilitate discharge for all referrals received. We aim to keep the transfer of care delays to a minimum for the benefit of both hospital inpatients and the Acute Trust whilst ensuring safe transfers with appropriate service provision.
Referrals into the team are usually identified via Multi-Disciplinary working on the wards, or direct referral to the team from ward staff.
The Transfer Team can be contacted on 0161 331 5183.
The team operates from 8.30am to 5.00pm Monday to Friday and 9.30am to 4.30pm on a Saturday.
Transfer team staff visit the wards daily and attend all Multi-Disciplinary Team meetings and or discharge meetings which are held on the individual wards.
