Welcome to Tameside and Glossop Community Specialist Palliative Care Service. This team provides specialist palliative care, advice and support to the people of Tameside and Glossop. The service is open to adults (over 18).
We visit patients in their own home and this includes people who live in nursing and care homes. Our team comprises of Macmillan Nurses who are specialists in pain and symptom control and the palliative care respite team who support patients and their families/carers within their own homes. This team can assist with personal care, respite sits and work alongside the district nurses in the provision of end of life care.
Every member of our team has received specialist training and education in palliative care. We work closely with general practitioners, district nurses and many other health care professionals in order to provide care for people within their own homes.
If you feel you would benefit from our service please discuss this with your GP or district nurse and they can arrange a referral.
