Health Mentors – Service Overview

Welcome to the Health Mentor Service. We are here to support young people aged 0-18 and their families.

 

About Our Service

For young people, we can offer more specific advice on issues that you may have to face as you get older.

These include:

  • Smoking
  • Healthy weight
  • Alcohol and drugs
  • Emotional health and wellbeing   

 

We are here if you need someone to talk to or if you need any help or advice about how to be healthy. We can visit you either in your school, in your local community or at home.

Our Expertise

We provide one to one support over 4-6 weeks using an agreed individual health plan to look at healthy behaviour changes.

How To Use The Service

The Health Mentor service can be accessed in all High Schools in Tameside via a drop in session. The sessions will depend on the individual school and details are available from the Health Mentor service or the school. Self-referral is available by completing the referral form online on our website.

We work as part of School Health teams, and work closely with School Nurses, Health Visitors and GP’s who can also refer you to the service.
