ADULT COMMUNITY PHYSIOTHERAPY SERVICE (WITH OCCUPATIONAL THERAPY)
The Adult Community Physiotherapy Team is based in Ashton Primary Care Centre and provides rehabilitation for people aged 16 or over who either live in Tameside and Glossop or who have a GP in Tameside and Glossop. (this excludes certain patients who live in Mossley as their physiotherapy services are provided by Pennine NHS FT.
To be accepted by our team for rehabilitation, you must require treatment at home because you are unable to access an out-patient facility.
People living in nursing or residential homes will be seen by our team.
We do not accept self-referrals; referrals to our team must be provided by a health or social care professional.
We also do not accept referrals which are solely for occupational therapy. This can be provided by the team if necessary and will be organised following your assessment by a physiotherapist.
The adult community physiotherapy service is provided Monday to Friday from 8am until 5pm (excluding Bank Holidays).
The team will visit anyone, meeting the above criteria with any condition which can be helped by physiotherapy. We generally do not visit people who have an acquired neurological condition as these people will be seen by the Community Neuro-Rehabilitation Team.
