NHS Tameside and Glossop Clinical Commissioning Group (CCG) has launched a consultation enabling residents of Tameside and Glossop to have their say on the way we deliver care for patients, usually older people, after leaving hospital or when they are at risk of being sent to hospital. This is known as Intermediate Care.
